About the role

The Wellness Navigator is responsible for providing support to clients in Cape May County that need mental health services. Work to support clients to obtain treatment services and maintain care to achieve wellness. Work with a team to ensure clients needing help to achieve mental wellness are wrapped with all needs of service. Educate clients regarding their health care condition and provide follow-up with clients and providers to ensure client needs are met. Operate as a strong community liaison to ensure that individuals can navigate and understand all services that are available to them for their mental health needs. Collaborate with Acenda and Community Providers to link individuals into services needed. As an integrated health organization Acenda values the use of evidence-based practice or EBP's.
