About the role
The Case Manager provides collaborative, court-involved community-based care by working closely with case managers, acute care teams, psychiatrists, hospitals, and the court system to support client wellness, long-term stability, and prevent rehospitalization. Engages clients in regular face-to-face and weekly sessions aligned with court-ordered treatment plans, coordinating mental health services, medications, family/support systems, and safety while maintaining timely, accurate court documentation. Communicates professionally and effectively with supervisors, referral sources, and external partners, demonstrating responsiveness, accountability, and commitment to agency standards and continuous improvement.
What you'll bring
- The individual must have some level of willingness and capacity to participate in the IOC program and plays an active role in the development of his/her treatment plan.
- The minimum requirement is a bachelor's degree in psychology, social work, or a related field, plus 1 year of experience with adults with mental illness.
- Must have and maintain a valid driver's license with an acceptable driving record.
