About the role

This role will aid clients in receiving integrated, client-focused care and support across all aspects of their treatment and recovery process. As a Case Manager, you will be instrumental in linking clients with the appropriate resources, enhancing skillsets, providing psychoeducation, monitoring their progress, and advocating for their mental health and physical health needs.

What you'll bring

• Bachelor’s degree in Social Work, Psychology, or related field
• Proven experience in case management
• Strong organizational and multi-tasking skills.
• Excellent communication and interpersonal skills, with an ability to interact effectively with diverse populations.
• Knowledge of community health resources and services.
