As a Hospice RN , you’ll act as a trusted Case Manager, delivering care that supports patients in their homes and communities. You will guide families through care decisions, while providing comfort, dignity, and compassionate support during life’s most tender moments. You will:
What you'll bring
- Current RN license in the State of Colorado
- Minimum of 2 years of nursing experience
- Valid driver’s license and reliable, insured vehicle
- Strong clinical skills and a passion for patient-centered care
- Ability to work independently and a passion for making a real difference
- Homecare Homebase (HCHB) experience preferred, not required
- hospice or home health experience preferred
