About the role

As the Associate Clinical Director (ACD) for CCM/PCM programs, you will support the Senior Director in driving operational excellence, quality assurance, and program performance across Chronic Care Management (CCM) and Principal Care Management (PCM) initiatives. This role blends operational leadership, data analysis, and clinical program oversight, ensuring consistency, compliance, and continuous improvement across teams and facilities.

What you'll bring

• Bachelor’s degree required
• 4+ years of experience in healthcare operations, care management, quality assurance, or clinical program oversight
• Strong understanding of CCM/PCM workflows, CMS documentation requirements, and compliance standards
• Experience with data analysis, reporting validation, and performance tracking
• Proven ability to identify process gaps and implement scalable improvements
• Excellent communication and cross-functional collaboration skills
• High attention to detail with strong organizational and analytical capabilities
• Experience working with EMRs and reporting tools preferred
• clinical or healthcare-related field preferred
