About the role

The RN Embedded Care Coordinator, Behavioral Health serves as a vital member of Foundation Counseling & Wellness, coordinating care and supporting patients seeking behavioral health services. This role partners closely with providers, patients, families, and community organizations to ensure timely access to care, effective care transitions, and a seamless patient experience. Using strong clinical judgment and a patient-centered approach, the RN Embedded Care Coordinator helps patients navigate behavioral health resources, supports care planning, assesses urgent needs, and connects patients with the appropriate level of care both within Southern New Hampshire Health and throughout the community.

What you'll bring

  • Associate Degree in Nursing (ASN) from an accredited nursing program
  • Current Registered Nurse (RN) license in the State of New Hampshire
  • Current BLS/CPR certification
  • Two (2) years of professional nursing experience in a relevant clinical specialty - Behavioral Health
  • Bachelor's degree in Nursing, Psychology, Sociology, Human Services, or a related field
  • Experience in ambulatory care, behavioral health, care coordination, case management, or population health
  • Certification in Ambulatory Care Nursing (RN-BC), Care Coordination (CCTM), or a related specialty certification
  • Strong assessment and clinical decision-making skills, including the ability to recognize and respond to behavioral health crises
  • Excellent communication, relationship-building, and conflict resolution abilities
  • Compassionate, patient-centered approach to supporting individuals with behavioral health needs
  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment
  • Ability to work independently while collaborating effectively with interdisciplinary teams and community partners
