This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO. • Active Illinois Physical Therapist (PT) license in good standing
• BLS CPR certification
• Reliable transportation for local patient visits
• Basic comfort with technology including email, texting, and EMR systems (training provided)
