About the role
As a Registered Nurse at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Health Partner, Enrollment Specialist, and Social Worker. You will deliver hands-on, compassionate care directly in patients’ homes and through virtual platforms. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. Your primary focus is delivering direct clinical care in the home: administering treatments, managing symptoms, conducting assessments, and being a trusted, present resource for patients and their families during one of the most challenging times of their lives. You will also support patients through virtual platforms and help them access Daymark’s services and support. In this role, you will provide direct patient care in the home and through virtual platforms. Additionally, you will conduct health assessments, educate patients, and deliver high-quality care to meet each patient's unique needs. What you'll bring
- A Bachelor of Science Degree in Nursing is required, along with a compact unrestricted registered nurse (RN) compact licensure in Rhode Island.
- Additionally, you have at least 3 years of nursing experience
- You are passionate about helping patients understand the benefits of palliative care.
- Availability: You can work with our team and patients full-time, five days per week.
- Strong Clinical Skills.
- You’re comfortable administering IV fluids, accessing ports when needed, and conducting point of care testing.
- In-Home Care Competency: Comfortable and effective working independently in patients’ homes, exercising strong clinical judgment without immediate on-site backup.
- Comfort with Technology.
- You’re comfortable using Slack, Zoom, Google Suite, and EHR systems.
- Strong relational skills.
- You have the ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
