About the role

Jaan Health is seeking dynamic and motivated Certified Medical Assistants (CMA) Clinical Care Coordinators to join our care team and will report to the Clinical Operations Manager. They will support the development of patient-centered, team-based care, as well as support primary care physicians (PCPs) and practices in managing their panel of patients using a population management informatics tool. This role focuses on high-volume patient outreach through calls, texts, and care platform messaging, while ensuring accurate documentation and seamless workflow with the care team.

What you'll bring

• Certified Medical Assistant required.
• Minimum of 3 years-experience in care management at a primary care setting or similar specialty.
• Excellent communication and technology skills.
• Prior use of electronic health records and other health care information systems required.
• Significant experience within a primary care setting.
• Proven problem-solver, comfortable with high-volume outreach, and ability to multi-task.
• Must have access to reliable internet and have a secure/private environment to work
• Experience in population health preferred.
• chronic care preferred, hospital, medical groups
