We’re seeking a dedicated Licensed Vocational Nurse (LVN) to play a pivotal role in our Palliative Care team. As a LVN with us, you'll be instrumental in delivering top-notch, evidence-based care to a panel of seriously ill patients. We're looking for someone with a genuine passion for this important work and a deep commitment to providing exceptional care to our patients. This position requires consistent travel throughout the below territory, with the primary travel occurring throughout North Valley, Sacramento, Solano, and Vallejo.
What you'll bring
- 2+ years of clinical nursing experience in home health or community health.
- Demonstrated proficiency in nursing practices, clinical judgment, and documentation.
- Strong oral and written communication skills.
- Strong decision-making skills and ability to work independently.
- Valid driver's license, insurance, and reliable vehicle.
- Proficiency in EMR, Google Suite, and other relevant technologies.
- Willing and able to work Monday through Friday 8:30am to 5pm Pacific Time, with flexibility for potential evenings and weekends.
- Direct experience in palliative care or hospice is highly preferred.
