About the role
CCareMore Health is seeking a Field Nurse Practitioner to provide acute, post-acute, and transitional care services to medically complex and recently discharged patients in their homes throughout the San Fernando Valley and Antelope Valley. As a Field Nurse Practitioner, you'll provide high-quality acute, post-acute, and transitional care services directly in patients' homes.
What you'll bring
- Master of Science in Nursing (MSN).
- Adult/Geriatric Nurse Practitioner certification.
- Active, unrestricted Registered Nurse (RN) license.
- Active, unrestricted Nurse Practitioner (NP) license in California.
- Minimum of one (1) year of Nurse Practitioner clinical experience.
- Strong assessment, diagnostic, and care management skills.
- Valid driver's license and reliable transportation.
- Ability to work independently while collaborating within a multidisciplinary team.
- Current TB test or willingness to obtain prior to employment.
