About the role
We are looking for an MSW to join our Hospice team. Come make a difference in the Cleveland area and surrounding communities! At Tennessee Home Health, our employees enjoy: The Medical Social Worker provides emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. A few position highlights include: Assist the patient and family with health related financial, social and emotional concerns and assist with effective coping skills. Provide social work services in accordance with the plan of care. Ensure that changes in the patient's clinical status are communicated to other healthcare team members and the agency in a timely manner.
What you'll bring
- Master's degree (MSW) or (BSW) from a school of Social Work accredited by the Council on Social Work Education.
- Must have a current and unencumbered Social Worker License in the state where services are rendered.
- Must have one (1) year of social work experience in a health care setting.
- Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
