What you'll bring

• RN-minimum of 1-3 years clinical experience required
• Minimum of 3 years of experience in diverse clinical setting is required
• LBSW, LMSW, LCSW
• BLS required
• 3-5 years of inpatient hospital case management experience preferred
• Vast experience in discharge planning preferred
• Local resources experience preferred
• Prior case management or discharge planning experience is preferred
• Prior experience with Cerner EMR is preferred
• Graduate of an accredited school of nursing with a minimum of an associate of science degree required
• bachelor’s degree preferred
