About the role
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team as a Start of Care (SOC) Nurse. The SOC Nurse is responsible for completing comprehensive patient assessments, developing individualized plans of care, coordinating services, and ensuring patients meet eligibility requirements for home health services. This role is ideal for an experienced nurse who enjoys independent practice, critical thinking, and patient-centered care in the home setting. Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related reasons.
What you'll bring
- Current Registered Nurse (RN) license in the State of Minnesota.
- Graduate of an accredited nursing program.
- Minimum one year of nursing experience required
- Strong assessment, communication, and organizational skills.
- Ability to work independently and manage time effectively.
- Valid driver's license, reliable transportation, and current auto insurance.
- OASIS experience preferred or willingness to complete training.
- home health experience preferred.
