Position Summary We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our growing in-office team in Houston, TX. This role is responsible for managing and coordinating the care of home health patients, ensuring high-quality clinical outcomes, regulatory compliance, and exceptional patient experiences. What you'll bring

• Prior Home Health Case Management experience required.
• Experience completing and reviewing OASIS assessments.
• Strong knowledge of Medicare Conditions of Participation (CoPs) and home health regulations.
• Experience coordinating care in a multidisciplinary healthcare environment.
• Excellent clinical assessment and critical thinking skills.
• Strong verbal and written communication skills.
• Proficiency with electronic medical record (EMR) systems.
• Ability to manage multiple priorities while maintaining accuracy and attention to detail.
• Strong organizational and time-management skills.
• Preferred Qualifications Five or more years of Home Health nursing experience.
• Previous experience as an RN Case Manager within a Medicare-certified home health agency.
• Proficiency with WellSky (Kinnser) software.
