About the role

Assists  the patients of the organization to overcome  seriocomic  barriers to achieving their health and wellness goals .   Ensures  access to quality healthcare for all individuals by  facilitating  access to community resources ,  referral services and life skill education  f or  patients  and their support systems.

What you'll bring

• Associate  degree in social services
• Two (2) or more years in social services or related field
• Knowledge of community resources relevant to social detriments of health
• Strong verbal communication skills, including excellent active listening skills
• Excellent written communication skills
• Ability to work harmoniously as part of a health care team
• Ability to meet approved minimal physical and medical standards
• Fluency in Spanish is a plus
• Bachelor’s  degree a plus
