About the role

We are seeking a dedicated Care Coordinator to serve as a consistent, non-clinical point of contact for patients and families throughout the cancer journey. This role is designed to reduce barriers to care, improve access to timely services, and provide practical and emotional support through close coordination with patients, providers, and interdisciplinary team members. As a small nimble team, this role will support vaccinations, in addition to care coordination.

What you'll bring

• · Bachelor’s degree
• · Experience in phlebotomy or Medical Assistant License
• · Strong communication and interpersonal skills, including empathy and cultural sensitivity
• · Proficiency in EMR use, phone communication, and task tracking systems
• · Comfort with change, process improvement, and working in a start-up or growing organization
• · Bachelor’s degree in social work, Public Health, Nursing, Health Education, or a related field
• · Oncology Patient Navigator Certification
• · Experience in patient navigation, care coordination, oncology, or a healthcare support role
• · Experience working with diverse populations and addressing social determinants of health
• · Team player who thrives in fast-paced, mission-driven healthcare environments
• · Detail-oriented and highly organized
• able to manage multiple patient caseloads and prioritize tasks
