About the role
We are looking for an RN to join our Hospice team. Come make a difference in the Clarksville, TN area and surrounding communities! At Beacon Hospice, our employees enjoy: The RN provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. A few position highlights include: Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes. To maintain continuity of patient care, plan and exchange information with physician, agency personnel, patient, family, and community resources. Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer. Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care.
