As part of the primary care treatment team, you will provide care coordination by working alongside our Community Health Workers, Medical Assistants, Nurses, Mental Health Case Managers, Counselors, Psychiatrists, Psych NP’s, and other team members to support continuity of care for patients with complex needs. Our integrated care model means our medical and behavioral health clinicians together help patients and their families address medical conditions and behavioral health factors that affect their health and well-being. What you'll bring

• MD/DO providers must have completed Primary Care residency (Family Practice, Internal Medicine, or Pediatrics) and be board certified.
• Licensed to practice in the State of Oregon with prescriptive authority and DEA
• Must be professionally bondable/insurable.
• Team-oriented with strong interpersonal and communication skills.
• Valid Oregon Driver's License and proof of automobile insurance.
• CPR certification in infant, child and adult required.
• Ability to function independently.
