About the role

As a Care Coordinator , you will support patients throughout the treatment process and serve as an important connection between the patient, treatment team, outside providers, and community resources.

What you'll bring

• The strongest Care Coordinators are compassionate problem-solvers who understand that long-term recovery often depends on successfully addressing the challenges patients face outside of treatment.
• Bachelor’s degree in social work, psychology, human services, behavioral health, or a related field required.
• Knowledge of behavioral health, substance use disorders, recovery principles, and trauma-informed care.
• Strong written and verbal communication skills.
• Strong organizational, documentation, and problem-solving skills.
• Ability to maintain confidentiality and professional boundaries.
• Current CPR and First Aid certification or ability to obtain within required onboarding timelines.
• Ability to successfully complete all required pre-employment screenings, training, and competency requirements.
• Minimum of two years of experience in behavioral health, substance use disorder treatment, healthcare, case management, or a related human-services setting preferred.
• Experience with intake coordination, case management, care coordination, or discharge planning preferred.
• Experience working in a substance use disorder treatment setting strongly preferred.
• Familiarity with ASAM criteria preferred.
