About the role
Transform home-based care—from wherever you are. As Circadia's full-time tele visit NP , you'll conduct remote assessments for home health patients, act on predictive health signals in real time, and coach care teams through virtual touchpoints to drive earlier interventions and reduce preventable hospitalizations. Please note that while the patient visits are conducted via Telehealth, this role is based out of our headquarters in El Segundo, CA, and requires being in the office 2-3 days a week. As a full-time Nurse Practitioner , you will be employed by Circadia Health, Inc. The base range is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
What you'll bring
- Certified and licensed Nurse Practitioner in CA
- ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation
- company bills on your behalf.
- 2+ years NP experience with complex adult populations in home health or post-acute settings
- able to practice independently.
- Comfortable conducting telehealth visits and navigating virtual care platforms.
- Thrives in a dynamic, evolving startup environment.
- English required.
- HCHB experience a plus
