About the role
Fall River Nephrology, a member of the Panoramic Health network, is seeking a compassionate and motivated Nurse Practitioner to join our growing nephrology team. This is an excellent opportunity to work alongside experienced nephrologists while caring for patients across outpatient clinics, dialysis centers, and hospital settings. The Nurse Practitioner will provide comprehensive care to patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), hypertension, and related conditions. The provider will collaborate closely with nephrologists, dialysis teams, hospital staff, and community providers to deliver coordinated, patient-centered care.
What you'll bring
- Master's or Doctoral degree from an accredited Nurse Practitioner program
- Current Massachusetts Nurse Practitioner license or eligibility for licensure
- Current DEA registration or ability to obtain
- Strong clinical assessment, diagnostic, and patient management skills
- Excellent communication and collaboration abilities
- Experience in nephrology, dialysis, internal medicine, hospital medicine, primary care, or chronic disease management
- Familiarity with electronic medical records
- Passion for caring for patients with complex chronic conditions
