About the role
Mon-Fri either 8am-5pm or 7:30am-4:30pm - hybrid with in-person patient meetings at our SWLR Clinic - Bachelor's & 1 year of work exp in related fields required - Bilingual strongly preferred The Community Health Worker (CHW) provides coordination of care for patients and families between ACH primary care providers and appropriate community resources. The CHW will help families navigate various systems of care while connecting them to the medical, social, and mental health services for which they may be eligible. CHWs serve as a liaison between the family, providers, and community based organizations (CBO). They should have a strong understanding of the healthcare delivery system and help patients overcome access challenges. The employee will provide coaching, support, and guidance to the patient as they work towards their health care goals.
What you'll bring
- Bachelor's degree from four-year college or university
- Or four years related experience and/or training; or equivalent combination of education and experience.
- Related Field - 1 year of experience
- Drivers License - Arkansas Department of Motor Vehicles
