About the role
The Case Manager shall perform the services below under the general direction of the Prince George's County Department of Corrections and the direct supervision of the Rebirth Substance Use Treatment (RSUT) Program Clinical Coordinator and Behavioral Health and Wellness Services Section Chief: Maintain all certifications and educational requirements that are necessary to provide clinical case management in the State of Complete program enrollment and screening/assessment tools. Develop person-centered continuity of care plans to outside services when appropriate and facilitate a continuum of care. Make appropriate referrals based on screening/assessment results.
What you'll bring
- Bachelor’s degree in Psychology, Criminology, Sociology, Public/Business Administration, Education, or closely related fields.
