• Active RN license in state of practice
• Graduate of an accredited nursing program
• Strong clinical judgment and ability to manage multiple priorities
• Confident communicator with the ability to collaborate across disciplines
• Comfortable working independently in a fast-paced, field-based environment
• Proven ability to manage complex patient cases and coordinate across teams
• Confidence making clinical decisions and driving outcomes
• Strong organizational skills with attention to detail in documentation and compliance
• Ability to proactively identify issues and take action
• A natural balance of accountability, leadership, and compassion
• Home Health experience strongly preferred
