About the role
The LPN Care Manager supports patients through telephonic outreach, care coordination, and ongoing health education. Responsibilities include monitoring care plans, scheduling appointments, reviewing medical records, documenting in EHRs, and facilitating access to medical services. Strong communication, organization, and computer skills are essential. What you'll bring
- New York LPN licensure is required.
- Maintaining a License is an ongoing requirement of this role
- Minimum of 2-years work experience in a healthcare setting involving patients
- Cronic Care Managment experience preffered.
- Proficiency with a personal computer
- Technical and analytical skill set
- Excellent communication, organizational, and interpersonal skills are necessary
- Compact LPN (Desired)
- Experience with Microsoft Excel preferred
