Job Description Spring Creek Rehab and Nursing is actively seeking a Director of Social Services for our Skilled Nursing Facility in Harrisburg, PA. The ideal candidate will have prior Long Term Care and managerial experience. Excellent compensation and comprehensive benefits packages provided! DUTIES: • Directs, establishes & plans the overall policies/goals for the Social Services Department • Responsible for assessments, care planning & helping residents adjust • Familiar with a variety of the field's concepts, practices & procedures • Relies on extensive experience & judgment to plan and accomplish goals • A wide degree of creativity and latitude is expected • Occasional Community Outreach • Meet with residents to discuss the financial plan • Gather documentation for Medicaid applications • Collect Private funds due to the nursing home • Assist with insurance authorizations Qualifications Additional Information All your information will be kept confidential according to EEO guidelines.
