About the role

We are seeking a compassionate and skilled LVN working on PRN basis to join our home health team for in-person, in-home health patient visits in Jourdanton, Pleasanton and surrounding areas. As an LVN on our team,  you will play a vital role in providing comprehensive nursing care to adult home health patients. You will work closely with the interdisciplinary team to ensure the highest quality of care. This is not a remote position, you must live in the Jourdanton, Pleasanton or surrounding area in order to see patients in person.

What you'll bring

• Valid LVN license issued by the State of Texas.
• You must live in the Jourdanton, Pleasanton, Poteet or surrounding areas.
• Minimum of one year of experience as an LVN providing in-home health patient visits.
• Experience using Kinnser/WellSky EMR, (or others), efficient with apps such as Slack and others.
• Current CPR BLS certification
• Strong clinical skills and knowledge of nursing principles and practices.
• Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.
• Professional, compassionate and sensitivite to the physical and emotional needs of our patients and their families.
• Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.
• Valid driver's license and reliable transportation for home visits.
• Commitment to maintaining confidentiality and upholding ethical standards in patient care.
