About the role

As a Hospice RN Case Manager, you will play a pivotal role in providing personalized, patient-centered care to individuals in their end-of-life journey, ensuring comfort, dignity, and peace. You will work closely with patients, families, and interdisciplinary teams to create and implement comprehensive care plans tailored to meet the unique needs of each individual.

What you'll bring

• Valid RN license in the state of practice.
• Minimum of two years of nursing experience
• Strong assessment and interpersonal skills to effectively communicate with patients and families.
• Ability to work independently and as part of a collaborative team.
• Compassionate and empathetic approach towards patient care.
• Knowledge of hospice regulations and best practices.
• Basic computer skills for documentation and communication purposes.
• preferably in hospice or palliative care.
