About the role
ChristianaCare HomeHealth is seeking a compassionate and relationship-driven Home Care Nurse Liaison (LPN) to serve as the key connection between referral sources, patients, families, and our HomeHealth team. In this role, you'll leverage your clinical expertise and communication skills to coordinate seamless transitions from hospital to home while ensuring patients receive the care and support they need. This role is Monday through Friday 8am-4:30pm with some 4 hour weekend days and holidays, depending upon the needs of the department. Join Our Team!
What you'll bring
- Current LPN license in the State of Delaware.
- Knowledge of HomeHealth services, practices, and care transition processes.
- Excellent communication, customer service, and relationship-building skills.
- Strong organizational, problem-solving, and computer skills.
- Ability to work independently in a fast-paced environment.
- Valid driver's license and active auto insurance required.
