About the role
This community-focused Registered Nurse (RN) will travel to members' homes across Massachusetts to complete in-person health assessments for a managed care health plan's dually eligible members. The field based role specializes in conducting timely, accurate assessments and documenting findings, then handing off to the health plan's care management team. This is assessment and documentation work, not care plan development or ongoing case management.
What you'll bring
- Active Massachusetts RN license (or higher, e.g., NP) in good standing required.
- Accredited, Associate Degree in Nursing required
- Valid driver’s license, reliable personal vehicle, and active auto insurance required.
- 1+ years of home health clinical experience required.
- Sound clinical judgment with strong organizational and decision-making skills.
- Comfortable working independently in members’ homes and managing a daily field schedule.
- Proficient with electronic documentation systems via secure virtual desktop environments.
- Ability to document accurately while managing multiple priorities while maintaining attention to detail.
- Proficient with standard office tools, and communication software.
- Strong interpersonal skills with the ability to build trust with members and families.
- Cultural sensitivity and awareness when engaging diverse populations.
- Vehicle & Licensing: Active driver’s license, reliable personal vehicle, and proof of insurance required.
