About the role
In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team the Case Manager will provide Intensive Case Management Services (ICMS) to clients recently released from jail who are experiencing homelessness. The Case Manager will assist with coordinating client’s release and help them attain permanent supportive housing in the community. The ODR Case Manager will work with clients to provide intensive field-based case management to a diverse group of clients with complex trauma facing medical, mental health and substance use barriers. Clients remain in the program for as long as they require subsidized housing.
What you'll bring
- or a minimum two years’ case management experience working with individuals experiencing homelessness
- Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record
- Able to transport clients in your vehicle
- Demonstrated knowledge of case management
- Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
- Skill in non-violent crisis intervention
- Computer literate
- able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs
- Detail oriented with strong time management, organizational, written, verbal, interpersonal, and computer skills
- Able to obtain and maintain CPR/First Aid certification
- Bachelor’s degree
- Minimum 30 WPM typing speed preferred
