About the role
As a Home Health Physical Therapist , you will work with senior care populations , including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings , helping patients regain independence and improve their quality of life. As a Physical Therapist in Home Health , you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients .
What you'll bring
- Licensed Physical Therapist (PT) – California (Required)
- CPR Certification (AHA approved)
- Minimum 1 year of clinical experience
- Strong clinical, communication, and patient education skills
- Valid driver’s license, reliable transportation, and auto insurance
- Experience in senior care, geriatrics, rehab, or community-based care preferred
- Home Health preferred
