About the role
Experience We're Looking For: Monarch is searching for a Nurse Practitioner or Physician Assistant for an Outpatient Primary Care position in both Wilson and Rocky Mount, NC. This will be a hybrid role of two days onsite at each location and one day remote from home. Work Schedule: Mon-Fri 8am-5pm. There will be occasional travel for home / community visits as needed for medical necessities averaging one day per month. The Primary Care Provider (Advanced Practice Provider – Physician Assistant or Nurse Practitioner) delivers high-quality medical care in the Monarch Healthcare clinic locations and within the community for home visits as needed. Responsibilities include conducting assessments, managing chronic conditions, prescribing and coordinating care, and promoting preventive health practices. The APP collaborates closely with Monarch Healthcare staff, behavioral health clinicians, and community partners to ensure integrated, whole-person and high-quality care is delivered. The position emphasizes outreach, relationship-building, and continuity of care to reduce barriers, improve wellness, and minimize avoidable hospital and emergency department use.
