About the role
The Patient Advocate & Liaison (PAL) is the face of Matter Health within the communities we serve. This field-based role is responsible for building trusted relationships with older adults living in affordable housing communities and helping them stay engaged in their healthcare journey. Working alongside an Nurse Practitioner (NP), you'll serve as the primary relationship-builder for your assigned communities—educating patients about Matter Health, supporting enrollment, coordinating follow-up, identifying barriers to care, and helping patients stay connected to the services they need. This role is ideal for someone who enjoys working directly with people, thrives in community settings, and is passionate about improving healthcare access for underserved populations.
What you'll bring
- 1–2 years of experience in community outreach, patient advocacy, healthcare, social services, customer service, hospitality, account management, sales, or other relationship-focused roles
- Passion for serving older adults and underserved communities
- Outstanding communication and relationship-building skills
- Comfortable working independently in a field-based environment
- Strong organization and time management skills
- Ability to collaborate with clinical and operational teams
- Valid driver's license and reliable transportation required
- Experience working with Medicare populations, community organizations, Salesforce, Athena, or healthcare outreach is a plus
- Bilingual Spanish is a plus
